Direct Access Physical Therapy in NY & NJ: What It Means for You

If you've ever put off physical therapy because you didn't have time to get a doctor's referral first, there's good news: in New York and New Jersey, you usually don't need one.

What "direct access" actually means

Direct access laws allow licensed physical therapists to evaluate and treat patients without a physician's prescription or referral. Both New York and New Jersey are direct access states, so if you're dealing with a nagging shoulder ache, a sprained ankle, or general pain that's been slowing you down, you can call and schedule a PT evaluation right away.

When direct access makes sense

Direct access is a great fit for musculoskeletal issues: sports injuries, overuse pain, postural strain, and many dance-related injuries fall into this category. Your therapist will still perform a full evaluation, including a health history and screening for red flags that might require a referral to a physician, imaging, or another specialist. The goal is to get you moving in the right direction faster, not to skip appropriate medical care.

What to expect at your first visit

Your initial evaluation typically runs 45–60 minutes and includes a review of your history, a postural assessment, movement screening, and hands-on testing specific to your complaint. From there, your therapist builds a plan of care tailored to your body, your sport, and your schedule, not a generic protocol.

A note on insurance

Direct access affects whether you need a referral to be seen. It's a separate question from what your insurance plan requires for coverage.
