What this site collects

E-Motion Physical Therapy & Wellness ("E-Motion," "we," "us") collects information you provide directly through forms on this website, including:

  • Appointment requests: name, email, phone, preferred location, preferred dates and times, reason for visit, and insurance carrier (optional).
  • Contact form submissions: name, email, phone, and your message.
  • Patient intake forms (sent only after an appointment is confirmed): demographics, emergency contact, insurance details, and health history you choose to provide.

How it's used

Information submitted through this site is used only to schedule, confirm, and prepare for your visit, and to respond to questions you send us. We do not sell your information, and we do not use it for advertising or marketing to third parties.

How it's handled technically

The appointment request and contact forms are processed through Netlify Forms, a third-party form-submission service, so a member of our front desk receives your request by email. Netlify Forms is not a signed HIPAA Business Associate. Please avoid including sensitive medical details in the free-text fields of these two forms — save those details for your intake form or your first visit, which are handled directly with our clinical team.

Cookies & analytics

This site may use a local, on-device theme preference (light/dark mode) stored in your browser. It does not currently use third-party analytics or advertising cookies.

Your choices

You can request a copy of the information we hold about you, ask us to correct it, or ask us to delete it, by contacting us using the details below. We'll respond within a reasonable time.
